This questionnaire has two purposes. The first is let us know what you’d like your will to say. The second is to alert us to any tax or other issues that might be raised by your will.
You will be emailed a copy of the questionnaire upon completion. Keep a copy of the completed form for your records. We will then prepare and send to you a draft will, and will work with you on the finalizing process.
In addition to wills, this questionnaire is designed to let us know if you would like two other documents prepared: a “living will” and a “durable power of attorney.” A Living Will, also sometimes referred to as “instructions to physicians,” is a statement made by you expressing your desire to be allowed to die instead of being kept alive by artificial means, in the event you are suffering from a terminal illness. A Durable Power of Attorney is a document that allows another person (usually your spouse) to transact business in the event you are incapacitated from handling matters on your own.
